Medication Incident
Any preventable circumstance or event that may cause or lead to inappropriate medication use or patient harm while the medication/IV fluid is in the control of the health care professional, patient or consumer.<br/><br/><b>Types of medication incidents </b><br/>NSIR includes potential and actual medication incidents related to prescribing, order communication, product labelling, packaging and nomenclature, compounding, dispensing, distribution, administration, monitoring, documentation and use. The reporting of medication incidents that have the potential to cause harm (i.e., reportable circumstances, near misses) is integral to facilitating the identification of processes that may require adjustments in order to prevent potentially hazardous and serious adverse outcomes. <br/>
